The Lifestyle Management Institute provides coordinated, comprehensive programs for the care of individuals with, or at risk for, chronic health problems such as heart disease, diabetes or osteoporosis. Services encompass prevention, detection, treatment and disease management programs for patients, families and the community. The institute also offers complementary therapies and health education programs. Our approach is to provide tools and to educate patients so they can achieve their goals for living a healthy life.

 

Your personal needs and lifestyle are considered so that we can develop a program tailored to you. Our interdisciplinary team of health and disease management specialists includes nurses, clinical exercise physiologists, clinical dieticians, certified diabetes educators, behavioral health specialists, social workers, pharmacists and complementary medicine practitioners. The institute's interdisciplinary team of specialists works in collaboration with you and your healthcare providers to reduce your health risks and enhance your quality of life.

All services have individual and group approaches to care offered onsite at the institute and in other community settings.

Insurance Coverage

 

Health screenings and support groups are available for a nominal fee or, in most instances, free of charge. Certain Health & Disease Management Service programs may require you to pay for the service. Major health insurance coverage varies. Check with your health insurance carrier to identify services eligible for reimbursement.
